So, in summary, it’s important to do a good history in the beginning.  Be familiar with your patient.  Have everything set up in advance.  Just use your patience, titrate the medications, keep an eye on them, recognize the complications early, and get some help, hopefully beforehand rather than in urgent circumstances.  Thanks everybody.
